Unlike many health conditions, diabetes can be
controlled by patients themselves when they understand and practice
proper medical management and lifestyle strategies.
Our Caritas Diabetes Care Program's outpatient services provide comprehensive education and support for participants and their families. From diet and exercise information to medication and glucose monitoring, Saint Anne's specially-trained health professionals partner with participants to help them learn to self-manage their diabetes and decrease the risk of future complications.
Nationally recognized by the American Diabetes Association, the Caritas Diabetes Care Program's certified diabetes nurse educator and registered dietitian/certified diabetes educator provide participants and their families:
Referral: A physician referral is required for diabetes outpatient services.
Insurance: Most health insurance plans cover all or at least a portion of this service. For those without insurance or whose coverage is insufficient, special payment arrangements can be made.
Interpreter Services: As with all hospital services, interpreters (any language) for non-English-speaking patients are available. Please call 508-674-5600, ext. 2455.
