Patient privacy is a priority at Norwood Hospital. Your medical record information is confidential and access is limited to the patient and authorized individuals. An authorization to release your medical record--also called protected health information (PHI)--must be signed by the patient, or the patient's legal representative (guardian or person with durable power of attorney). If a child is an emancipated minor or age eighteen years or older, parents cannot get copies of their child’s medical record.
