Clinical and Nursing Services

To ensure that you have the best care possible, we provide comprehensive in-home care. Our clinicians act as care managers overseeing your care and working with your physicians to provide for all of your needs. Upon admission, we complete a full assessment of the patient and their medical needs in the home setting.

We offer complete nursing and therapy services. 

Nurses provide quality care in:

• Dressing changes, wound treatment and ostomy care
• Diseases such as diabetes, CHF, COPD, cardiac disease, respiratory conditions
• Care Management in partnership with your physicians
• Mental health and mental illnesses
• IV therapy for hydration, medication, nutrition, and chemotherapy
• Terminal illness
• Adult, mommy-baby, and child health  

Physical, Occupational and Speech therapists provide:

• Home rehabilitation services
• Development of disease management  programs to meet the needs of patients and their families.

Certified Home Health Aides provide:

• Assistance with personal care: bathing, dressing, shaving, etc.
• Assistance with therapy programs along with nurses and therapists

• Counsel to patients and families regarding emotional difficulties related to illness and the resulting financial burdens
