Unlike many health conditions, diabetes can be controlled by patients themselves when they understand and practice proper medical management and lifestyle strategies.
Our Caritas Diabetes Care Program's outpatient services provide
comprehensive education and support for participants and their
families. From diet and exercise information to medication and
glucose monitoring, Saint Anne's specially-trained health
professionals partner with participants to help them learn to
self-manage their diabetes and decrease the risk of future
complications.
Nationally recognized by the American Diabetes Association, the
Caritas Diabetes Care Program's certified diabetes nurse
educator and registered dietitian/certified diabetes educator
provide participants and their families:
• Individualized assessment by a certified diabetes
educator
• Instruction in self blood glucose monitoring and insulin
administration
• Nutrition assessment and recommendations by a registered
dietitian/certified diabetes educator
• Small group or individual classes that cover such topics
as:
* healthy eating
* activity and medication in glucose control
* the emotional component of living with diabetes
* travel planning
* dining out
* food preparation
* problem-solving
• Follow-up telephone call and office visits
• Individualized nutrition and nurse consultations
• Monthly diabetes education and support groups
Referral: A physician referral is required for diabetes
outpatient services.
Insurance: Most health insurance plans cover all or at least a
portion of this service. For those without insurance or whose
coverage is insufficient, special payment arrangements can be
made.
Interpreter Services: As with all hospital services,
interpreters (any language) for non-English-speaking patients
